What is My Health Portfolio?

My Health Portfolio is a place where you can store your health data. It also lets you organize, view and edit the information that you’ve entered on the Revolution Health website, as well as makes it easier to keep track of information as it changes over time. You can enter new information at any time and print out the saved information.

What is the purpose of the identification section of My Health Portfolio?

The identification section includes personal information such as name, home address, email address, employer name/address and phone numbers. This information is required when you visit a doctor or other health care provider.

Can I create printouts of information in My Health Portfolio?

Yes. The “Print Your Records” feature on the right-hand side of the Health Portfolio main page (My Revolution > My Health Portfolio) lets you create a PDF document that you can save and review whenever you want. You’ll need to create a title for the PDF before you view, download or print it. You can also specify the section of your Health Portfolio that you want included in the PDF by checking the box in the column titled “Include in printout?”

Do I have to fill out all of the requested information in My Health Portfolio?

No. You don’t need to fill out all of the requested information in your Health Portfolio. We’ve created a portfolio with separate sections, and you can choose how much information to fill out. Your records become more complete when you enter more data. For more information, see the question below on how the completeness value is calculated.

How is the completeness value for each section calculated in My Health Portfolio?

Some fields in your Health Portfolio are considered more pertinent than others and impact the completeness value (”Started,” “Complete” or “More detailed”). For a section of your portfolio to be considered complete, fill out all fields noted by the icon “Recommended for a complete record.” For it to be “More detailed,” fill out the fields that are not required for a complete record. The completeness levels are:

Can I tell when the different sections of My Health Portfolio were updated?

Yes. There’s a column titled “Last updated” on the main My Health Portfolio screen that indicates the last time you saved a section. Even if your information has not changed, you can edit and save the section to update the date.

Can I print a section of My Health Portfolio if it is not yet complete?

Yes. You can print out any section of your Health Portfolio even if it’s not yet complete.

What is the purpose of the Basic Health Information section of My Health Portfolio?

The Basic Health Information section lets you enter information that’s usually required during a medical visit, such as your date of birth, height, weight, gender, ethnicity/race and relationship status (single, married, etc.). You can also enter information such as blood type, cholesterol and blood pressure.

What information can be entered in the Emergency Contacts section of My Health Portfolio?

The Emergency Contacts section in your Health Portfolio lets you give the name, address, phone numbers and relationship of your primary contact. You can enter a second contact as well as contact information for your key providers.

Can I enter and save the names, phone numbers and addresses of the doctors that I’ve seen?

Yes. The Doctors and Providers section in your Health Portfolio lets you enter information such as multiple doctors or providers names, their specialties, practice names, and addresses and phone numbers. Go to My Revolution > My Health Portfolio and click on the Doctors and Providers section to enter your information.

Can I enter and save my medical conditions?

Yes. The Medical Conditions section in your Health Portfolio lets you specify your medical conditions. Select the condition(s) that you have and the age at which the condition began. Go to My Revolution > My Health Portfolio and click on the Medical Conditions section to enter your medical conditions.

Can the medical condition(s) I’ve entered in My Health Portfolio be seen by other people?

No. Adding your medical conditions to your Health Portfolio won’t make them automatically accessible to others. You have complete control over who can view what information. If you want your conditions to be viewable to others, you must specify this by going to My Revolution > My Profile and creating your profile. In your profile you can select which conditions to share, if any, and with whom you want to share them. For more information about setting privacy controls on the health conditions that you included in your profile, go to the Profile FAQs.

Can I enter and save my family medical history?

Yes. The Family History section in your Health Portfolio lets you specify if anyone in your family has had certain diseases or conditions. You can also select the disease(s) or condition(s) that have affected one or more family members. To enter your family medical history, go to My Revolution > My Health Portfolio and click on the Family History section.

What is the difference between My Health Portfolio and My Public Profile, and why would I enter medical conditions in both?

Your health portfolio is your secure personal health record and is used to store all of your medical information electronically for easy access when you need it. This information can not be seen by other people and is visible only to you. Think of it as your private health lock-box. Your Public Profile, which can be found in My Revolution > My Public Profile, is used to enter information about yourself that you may want to share with other people in the Revolution Health community. This information may include your medical conditions, conditions of someone else you care about, or your general health interests. You can choose with whom you want to share your profile information: everyone, a specific group of people (your circle), all Revolution Health users, or just you. Entering information in your profile, makes it easier to find people with similar interests as you and is your face to the Revolution Health community.

Where can I keep basic health information about myself?

Go to My Revolution > My Health Portfolio and go to the Basic health information section to enter and save your basic health information.
